How We Determine Eligibility
This database covers 160 items across four eligibility categories. Understanding how each category is determined — and who makes the final call — will help you use your HSA more confidently.
All items marked as automatically eligible are sourced from IRS Publication 502 (Medical and Dental Expenses), IRS Publication 969 (HSAs and Other Tax-Favored Health Plans), and the CARES Act of 2020 — the primary IRS references used to determine HSA-qualified medical expenses.
No documentation is needed for automatically eligible items.
Items marked as HSA admin dependent are not clearly defined in IRS guidance. Eligibility for these expenses is determined by your HSA administrator based on their plan rules, internal interpretation of IRS policies, and real-world approval practices — always contact your administrator to verify before purchasing.
Items marked LMN Required are not automatically covered per the IRS, but can qualify with a Letter of Medical Necessity (LMN) from a licensed healthcare provider. The LMN must connect the expense to a specific diagnosed medical condition, treatment, or prevention plan. Refer to your HSA Administrator to determine eligibility and LMN requirements.

Items marked Not Eligible generally do not qualify as HSA-eligible expenses under current IRS guidance and are declined by most administrators. These are expenses the IRS considers personal, cosmetic, or general health and wellness in nature — not tied to the diagnosis, treatment, or prevention of a specific medical condition.
Note that in some cases, a Letter of Medical Necessity (LMN) may allow an otherwise ineligible item to qualify if a licensed provider can document a direct medical connection. However, this is not guaranteed and varies by administrator.
