Are Apple Watches HSA eligible? It depends — and the difference can mean hundreds of dollars. Here's exactly when you can (and can't) use your HSA for an Apple Watch.

Can you buy an Apple Watch with HSA funds?

Maybe — an Apple Watch purchased as a general consumer product is not HSA eligible. However, it can qualify under specific circumstances: if your doctor prescribes it to diagnose or treat a specific medical condition, or if you use it primarily to monitor a diagnosed condition (like AFib or irregular heart rhythm). Without a medical purpose tied to a diagnosis, the IRS considers it a general wellness device.

What the IRS States

The IRS governs medical and HSA eligibility under Publication 502 and Publication 969. The key test: an expense must primarily serve to diagnose, cure, treat, mitigate, or prevent a specific medical condition — not just promote general health or wellness. In fact, "general wellness" is explicitly excluded.

This is where Apple Watch gets complicated. It's marketed as a health and fitness device — tracking steps, calories, heart rate, and sleep — which puts it squarely in the general wellness bucket. However, newer models include FDA-cleared medical features like the ECG app and irregular rhythm notifications that can serve a genuine clinical purpose. A product with both medical and general wellness uses can still qualify, but only if the primary purpose is treating, mitigating or preventing a diagnosed condition.

This is where a Letter of Medical Necessity (LMN) comes in. An LMN is a signed document from a licensed healthcare provider stating that a specific product or service is medically necessary to treat, mitigate, or prevent a diagnosed condition.

For a dual-purpose device like an Apple Watch, an LMN is what bridges the gap between "general wellness gadget" and "legitimate HSA expense" — it provides the documentation your HSA plan administrator needs to approve the purchase and protects you in the event of an audit.

When an Apple Watch Qualifies — and When It Doesn't

✅ Likely Eligible

❌ Not Eligible

Doctor prescribes it to monitor a diagnosed health or heart condition such as AFib. LMN is obtained.

Purchased for fitness tracking, step counting, or general health

Your HSA plan explicitly lists wearables as eligible expenses

Bundled with a gym membership or fitness apps, signaling general wellness use

⚠️ Important Warning

Always keep documentation: a Letter of Medical Necessity (LMN) from your doctor (or provider such as Truemed) and records showing the device is used for a specific medical condition. Using HSA funds for ineligible expenses results in taxes plus a 20% penalty.

How to Purchase an Apple Watch with HSA Funds

If you have a legitimate medical reason to use an Apple Watch — especially for heart monitoring — here's how to document the purchase properly:

Apple Watch Series 11

Available on Amazon

1: Confirm with Your HSA Administrator

Before purchasing, contact your HSA plan administrator and ask if the Apple Watch is eligible with an LMN. Some plans allow it; others may require pre-approval. Get the answer in writing with what information is required in the letter.

2: Get a Letter of Medical Necessity (LMN)

Ask your doctor to write an LMN stating that an Apple Watch is medically necessary to monitor or manage a specific diagnosed condition (e.g., atrial fibrillation, heart arrhythmia). The letter should name the diagnosis and explain why this device is part of your treatment.

3: Keep All Documentation

Save your LMN, the receipt, and any correspondence with your HSA administrator. The IRS requires you to substantiate HSA withdrawals if audited, and the documentation window can extend years after the purchase.

4: Use Your HSA Card or Get Reimbursed

Pay with your HSA debit card at checkout, or pay out-of-pocket and submit for reimbursement. Either way, your documentation protects you if you're ever audited.

Frequently Asked Questions

Can I use my HSA card to buy an Apple Watch on Apple.com?

Apple.com doesn't screen for HSA eligibility at checkout — your HSA card may process the transaction. However, using the card doesn't make the purchase eligible. If the purchase doesn't meet IRS requirements, you'll owe income tax plus a 20% penalty on the amount withdrawn. Always confirm eligibility before using HSA funds.

Is Apple Watch eligible under an FSA (Flexible Spending Account)?

FSA and HSA eligibility rules follow the same IRS guidelines, so the same standard applies: an Apple Watch is generally not FSA eligible unless tied to a specific medical diagnosis with proper documentation. Some FSA plans have slightly more flexibility, so check with your plan administrator.

What about Apple Watch Series 9 or Apple Watch Ultra 2?

The model doesn't change the eligibility analysis. What matters is how you use the device and whether it's medically prescribed. The Apple Watch Ultra 2's more advanced health sensors don't automatically make it HSA eligible — you still need a medical purpose and documentation.

Can I get reimbursed from my HSA for an Apple Watch I already purchased?

If you paid out-of-pocket and had a valid medical reason at the time of purchase (with an LMN), you can submit for reimbursement. Retroactive reimbursements must still meet eligibility requirements. You cannot create a medical purpose after the fact — the prescription should predate or coincide with the purchase.

Are Apple AirPods or other Apple health products HSA eligible?

AirPods are generally not HSA eligible — they're communication devices. However, hearing aids are a clear HSA-eligible expense. If your doctor prescribes an over-the-counter hearing device and AirPods are recommended in that capacity, there could be an argument — but this is highly situational and requires professional tax guidance.

The Bottom Line

Buying an Apple Watch with your HSA is not a straightforward "yes." For most people buying it for everyday health and fitness tracking, it is not HSA eligible. The IRS's general wellness exclusion is clear.

However, if your doctor prescribes an Apple Watch to help monitor a diagnosed heart condition — particularly using the FDA-cleared ECG features — and you have a Letter of Medical Necessity in hand, you have a defensible case for HSA eligibility. Always loop in your HSA administrator and keep your documentation airtight.

When in doubt, stick to the clearly eligible purchases and save your HSA funds for expenses that won't put you at risk of penalties.
